Alabama’s Hospital to Home Program (H2H) is designed to support transitions of eligible Medicaid recipients back to the community after a hospital stay. The program utilizes an expedited referral and application process that allows services to begin immediately upon discharge. The program is designed to help eligible patients return to and safely remain in the community, thereby reducing long-term care placement and hospital readmission rates.
H2H works in coordination with the Alabama Gateway to Community Living (GCL) and Alabama Community Transitions (ACT) Programs which are designed to transition individuals from facilities back to the community. This model, established by the Alabama Medicaid Agency, will integrate the existing Integrated Care Network's Case Management Organizations (CMOs) with health systems in order to provide expedited access to the ACT or Elderly and Disabled Medicaid Waiver services for Medicaid recipients who upon discharge are at risk of institutional long-term care placement. This diversion strategy is part of an effort to streamline access to long-term services and support for patients and families.
The program is available to full eligible Medicaid recipients in select Alabama Counties.

Alabama’s Hospital to Home Program (H2H) is designed to provide assistance to hospitals in transitioning certain Alabama Medicaid recipients who have been admitted to an acute care facility back to their homes instead of a nursing facility.
The usage of Alabama's Hospital to Home Referral System is currently limited to hospital personnel. First time hospital users must register for an account prior to using the H2H Referral System. Select the Register link from the top menu to complete the registration process. New users will receive a confirmation email once their registration has been processed. After receiving confirmation, users may log into the H2H Referral System by selecting Login from the top menu and entering their account credentials.
